2. Select Donation Amount. ($200/$400/OTHER)
3. Name, Email, and Address Information
4. Tax Credit Purpose. SELECT: Extra-Curricular Activities
5. Extra-Curricular Activity Type. SELECT: Athletics
6. Name of Club/Activity. ENTER: Mountain Bike Team (IMPORTANT! You must enter this in order for the team to benefit from your donation.)
7. How did you hear about Us? ENTER: Other
8. Enter your payment details.
9. DONATE NOW!
